Seorang detektif swasta yang sekolah di SMU Teitan bernama Sinichi Kudo telah banyak membantu polisi untuk memecahkan berbagai kasus. Dari mulai kasus pembunuhan, pencurian ataupun penculikan. Suatu ketika ketika Shinici Kudo pergi kencan dengan pacarnya Ran Mouri, mereka menyelesaikan sebuah kasus pembunuhan yang terjadi di taman hiburan. Dan salah satu tersangkanya adalah 2 orang berjubah hitam yang diketahui bernama Gin dan Vodka. Kemudian setelah memecahkan kasus itu, Sinichi mengikuti dua orang tersebut.

Dan dia pun akhirnya diketahui dan dipukul sampai pingsan. Saat itulah 2 orang misterius itu memberikannya obat aneh, yang diketahui namanya adalah Apotoxin-4869 (APTX-4869).

Dan ketika Sinichi siuman tubuhnya diketahui telah menyusut menjadi seorang anak kecil.
